- Hospital-Based Rehabilitation Program: Inpatient care for acute symptoms, along with therapy, medication management, and transition planning, ensures a smooth return to daily life. This level of care is for individuals who require 24-hour support and monitoring due to severe symptoms or a recent hospitalization.
